Use these forms to coordinate chapter activity and to receive financial support from our national office.
- Request experts and practitioners for your campus chapter with our speaker request form 6-8 weeks before your event.
- Inform us of your event with the advance event notification form two weeks before.
- Fill out the post-speaker event form to get reimbursed and credit for your event.
- Complete the non-speaker event reimbursement form for all chapter expenses not associated with speaker events.
Promote AHS on Campus
