Speaker Event Forms
Speaker Request Form (6-8 weeks before): Request a speaker for your campus event, 6-8 weeks prior to the event date. After you complete the invitation, we will send it to the speaker and make every effort to get a timely response. You should hear from us within one week. When the speaker has accepted your invitation, we will put you in direct contact. At that point, you will become the speaker's primary contact, responsible for keeping the speaker informed and for responding promptly when the speaker gets in touch with you.
Advance Event Notification (2 weeks before): It is very important that we know about events as soon as you have scheduled them. This form should be filled out at least 2 weeks before the event date. You must complete this document to be reimbursed after the event.
Speaker post-Event Form: Please fill out the post-event form within 4 weeks of your speaker event. Attach any high quality pictures, video, campus newspaper articles, and contact information you collected from student attendees to the PEF. You must complete this report to be reimbursed. Submit your receipts as a single attachment on the PEF. You can either scan and save as a PDF or take pictures and combine them into a single document.
Non-Speaker Post-Event Form
Non-Speaker Post-Event Form: If you organized a non-speaker event, such as an open house, movie night, trivia, etc., please submit this form within four weeks of the event.
If you need reimbursements, submit your receipts as a single attachment. You can either scan and save as a PDF or take pictures and combine them into a single document.
Semesterly Roundtable Discussion Report
Semesterly Roundtable Discussion Report. Please complete this short form regarding your chapter's group discussions for the semester. We will use your report to tabulate chapter points, and to improve our support for your chapters' programming proposals.
